How do I get my child’s passport with one parent absent?
If one parent/guardian cannot go with the child to apply for the passport, they can give permission by completing Form DS-3053 “Statement of Consent.” You must submit the form to the U.S. Embassy or Consulate in the country where you are applying for a passport.
If you do not have a parent or guardian with you when you apply, you will be required to fill out an application form. The form must be signed by both parents or guardians. You will also need to provide a photocopy of your birth certificate or other official document that shows your parent’s name and date of birth.
Do I need my daughters father’s permission for a passport?
